Course Objectives

Upon successful completion of this module, you will be able to:

  • Describe the anatomy and physiology of the cardiac conduction system and explain how the coordinated flow of ions across myocardial cell membranes generates the electrical signals recorded at the body surface.
  • Identify the ten electrodes used to record a standard 12-lead electrocardiogram, state the correct anatomic placement of each, and explain how the electrodes are combined to form the twelve leads and the electrical views they provide.
  • Distinguish the three groups of leads—the bipolar limb leads, the augmented unipolar limb leads, and the precordial leads—and relate each lead or lead group to the region of the heart it interrogates.
  • Identify and measure the normal waveforms, segments, and intervals of the electrocardiogram, including the P wave, QRS complex, T wave, U wave, PR interval, QRS duration, ST segment, and QT interval, using standardized paper speed and calibration.
  • Apply a systematic, reproducible method for interpreting any 12-lead tracing, including determination of heart rate, rhythm, frontal-plane axis, intervals, and waveform morphology, and use that method to recognize a normal electrocardiogram.
  • Recognize and correctly name the electrocardiographic features of asystole, atrial fibrillation, atrial flutter, sinus bradycardia and tachycardia, premature atrial contractions, premature ventricular contractions, ventricular tachycardia, and ventricular fibrillation.
  • Recognize the electrocardiographic manifestations of myocardial ischemia and infarction, including angina, non–ST-elevation and ST-elevation myocardial infarction, and localize the region of injury and the likely culprit coronary artery from the pattern of lead involvement.
  • Recognize the electrocardiographic patterns associated with chamber enlargement, bundle branch and atrioventricular block, congestive heart failure, and chronic obstructive pulmonary disease and cor pulmonale.
  • Correlate the physical signs and symptoms of cardiopulmonary disease with the corresponding electrocardiographic findings, and appreciate the limitations, artifacts, and pitfalls that affect accurate interpretation.
